Please label your file &#8220;Last_MLMLDiveApp_Year&#8221; for it to be considered. If you have completed an AAUS Scientific Diving Course at another institution, please refer to <a href=\"https:\/\/mlml.sjsu.edu\/diving\/transferring-certifications\/\">transferring certifications<\/a>.<\/p>\n<h3 class=\"widget-title\">Required Documents<\/h3>\n<p><em>Please label each file as &#8220;LastName_DocumentName_year&#8221; e.g. &#8220;Steller_DANInsurance_2025&#8221; and upload into your shared google drive folder.<\/em><\/p>\n<ol>\n<li><strong><a href=\"https:\/\/docs.google.com\/forms\/d\/1vuM51n3azATfa2jcXsyrZvWFtQMnIbVSHQALPjLbvIE\/edit\">Diver Information Application<\/a><\/strong> &#8211; Completed, including a summary of diving experience and most recent time on unassisted, 400yd swim.<\/li>\n<li><strong>Certification cards<\/strong> &#8211; copy all SCUBA certification cards, including DAN membership, DAN insurance coverage, CPR, First Aid, and Emergency Oxygen.<\/li>\n<li><a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2022\/03\/Dive-Medical-3_2022.pdf\"><strong>Medical Exam<\/strong><\/a> &#8211; A current diving medical is required to dive with MLML&#8217;s research diving program. Anyone interested in diving must obtain a diving medical from a licensed physician before they can SCUBA dive&#8211;ideally with a physician familiar to some degree with the physical demands of SCUBA diving. The student Health Center on consortium campuses can usually perform the required tests at reduced costs, but needs to be scheduled several weeks in advance. If possible, complete this medical exam prior to arriving at MLML.\n<ul>\n<li>Current MLML divers must have a new medical done every 5 years until age 40, every 3 years after age 40 until age 60, every 2 years after age 60, and after any diving accident.<\/li>\n<li>Additional testing is required for divers over the age of 40 and can be found in the medical exam.<\/li>\n<li>Additional testing may be required if prior COVID infection has occurred.<\/li>\n<li>APPROVAL MUST BE ON THIS AAUS MLML FORM.<\/li>\n<li>Diver must sign the release and verify that a <strong>medical practitioner and not a physician assistant<\/strong>, signed off on the form. To comply with HIPAA guidelines, we require you to ONLY upload pages 1-4 of the medical exam as a PDF to your shared documents folder. Pages 5-11 include private medical information &#8212; please do not share this with us.<\/li>\n<\/ul>\n<\/li>\n<li><a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2017\/12\/MLML-Dive-Waiver-1.pdf\"><strong>MLML Dive Waiver<\/strong><\/a> &#8211; Read it, sign it, date it, and <strong>have it witnessed<\/strong>.<\/li>\n<li><a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2017\/12\/MLML-Dive-Log-Template.xls\"><strong>MLML Dive Log<\/strong><\/a> &#8211; Provide proof of past diving activity, and a log of the <strong>12 most recent dives<\/strong> on an MLML dive log form.<\/li>\n<li><a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2020\/03\/MLML_AAUS_Scientific_Diving_Standards_Manual_2020.pdf\"><strong>MLML Standards for Scientific Diving<\/strong><\/a> &#8211; Read the MLML Standards for Scientific Diving manual, then download and complete the <a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2017\/12\/MLML_Manual-AgreementForm.doc\"><strong>MLML Manual Agreement Form<\/strong><\/a>, signifying that you have read the regulations and listed emergency contact information.<\/li>\n<li><a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2008\/09\/MLML_Computer_guidelines.pdf\"><strong>MLML Computer Guidelines<\/strong><\/a> &#8211; To use a computer for diving at MLML you must have a copy of the users manual, show proof of knowledge for use.\u00a0Then complete <strong><a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2020\/06\/MLML-Dive-Computer-Quiz.docx\">Dive Computer Quiz<\/a><\/strong> of your understanding.<\/li>\n<li><a href=\"https:\/\/mlml.sjsu.edu\/diving\/wp-content\/uploads\/sites\/42\/2017\/12\/New-MLML-Equipment-Form.pdf\"><strong>Equipment<\/strong><\/a> &#8211; <strong>Proof of annual servicing (regulator &amp; BCD) is required at time of entry into the program. <\/strong>Obtain a copy of the servicing records from your dive shop. If gear is &lt; 1 year old, proof of purchase is an acceptable alternative. A check of your diving gear will be made before the open water dives.\u00a0<strong><br \/>\n<\/strong><\/p>\n<ul>\n<li>Required equipment includes:\n<ul>\n<li>ditchable weight system (ex. belt, integrated pockets,harness) <em>and<\/em> your own personal weight (10% of your body weight + 4-6 lbs)<\/li>\n<li>buoyancy compensator (with whistle and appropriate clips for avoiding dangling gear)<\/li>\n<li>regulator <span style=\"font-weight: 400\">&#8211; must have a primary 2<\/span><span style=\"font-weight: 400\">nd<\/span><span style=\"font-weight: 400\"> stage, and an octopus, LP inflator hose and gauges including an SPG.\u00a0 <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span> <span style=\"font-weight: 400\">If BCD has an integrated air source, divers must also carry an octopus for \u2018COVID-safer\u2019 and redundant air sharing.\u00a0 <\/span><span style=\"font-weight: 400\"><br \/>\n<\/span> <span style=\"font-weight: 400\">All divers must have an analog SPG.<\/span><\/li>\n<li>mask &amp; snorkel<\/li>\n<li>7mm wetsuit minimum<\/li>\n<li>5mm gloves<\/li>\n<li>5mm or 7mm booties<\/li>\n<li>5mm hood (omit if hood is integrated with wetsuit)<\/li>\n<li>timing <span style=\"font-weight: 400\">device (minimum water resistant to 50m), depth gauge and dive tables OR dive computer + manual<\/span><\/li>\n<li>compass<\/li>\n<li>cutting tool (safety shears or dive knife with strap)<\/li>\n<li>equipment bag (that fits all of your gear)<\/li>\n<li>smaller mesh goodie bag (that fits mask, fins, and snorkel) for boat diving<\/li>\n<li>personal save-a-dive kit<\/li>\n<li>dive light (minimum one, preferable to have 2)<\/li>\n<li>gear storage container (ex. Action Packer Tote \/ Tough Storage Tote)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li><strong>Written Examination<\/strong> &#8211; A thorough written exam must be completed. This is given during the class.<\/li>\n<li><strong><a href=\"https:\/\/mlml.sjsu.edu\/diving\/skills\/\">Pool Evaluation<\/a><\/strong> &#8211; All pool tests <strong>must be passed<\/strong> in order to continue in either course.
